Clubs should note all information sent from the South West Administration Office is sent to the Club's Nominated League Contact. If you have more than one team remember to complete one of these screens for each team.

As material is sent all year round it is suggested a home address is given rather than a clubhouse as important items of mail (including fixtures and registration forms may be lost).
